Appointment Only
All three immunization clinics are seeing patients primarily through scheduled appointments to allow proper social distancing in the clinic. Walk-ins will be accommodated as able and are not guaranteed to be seen. Scheduled appointments will have priority. Please call 602-506-6767 to schedule.  


Prepare for Your Visit

  • Please bring the immunization record for the person getting the vaccine(s), insurance card (if insured) and a pen. 
  • To help protect the health of others, adults and children over age 2 are asked to wear a cloth face covering when visiting our clinics.

REMEMBER: Bring your insurance card and immunization record to every visit.

Why Immunize?

Immunizations have proven to be the most effective way to stop the spread of disease. Here in Maricopa County your child from age 0 years up to their 19th birthday is encouraged to receive childhood immunizations from either his/her health care provider or from one of the Maricopa County Department of Public Health childhood immunization clinics.
