COVID-19 vaccine unavailable: We currently are not providing COVID-19 vaccination at any of our clinics until the 2023-24 updated COVID-19 vaccines are distributed. You can find them from providers by entering your ZIP code on Vaccines.gov or calling our CARES Team 602-506-6767 for assistance.

Please bring the immunization record for the person getting the vaccine(s), insurance card (if insured) and a pen.
To help protect the health of others, adults and children over age 2 are asked to wear a face mask when visiting our clinics.

Immunizations have proven to be the most effective way to stop the spread of disease. Here in Maricopa County your child from age 0 years up to their 19th birthday is encouraged to receive childhood immunizations from either his/her health care provider or from one of the Maricopa County Department of Public Health childhood immunization clinics.
