Personal Care/Home Health Aid


The SAIL Case Management program helps individuals live independently in their homes by coordinating needed assistance with daily living activities. The program provides case management services to individuals age 60 and older, disabled adults, and to families and caregivers. Emphasis is on serving those in greatest social and economic need who will benefit from the services.
 
SAIL Case managers conduct in-home evaluations to determine an individual’s area of need and make referrals to an agency or program that may provide additional support and assistance to the individuals.
Case managers coordinate in-home services that are provided by other agencies. One of these services is Personal Care / Home Health Aide.

Individuals with difficulty bathing or personal grooming may be eligible for this service which is funded by the Area Agency on Aging, Region One.  An individual is assessed by the SAIL case manager and eligibility is determined.  If determined eligible for Personal Care/Home Health Aide, a provider agency will come to the home on a set schedule to assist the individual with bathing and personal grooming.  This service is generally provided weekly.
Per Personal Care/Home Health services are generally provided at no cost to the individual, but a cost share for the service may be required by the Area Agency on Aging, Region One.
